Sport Physicals Will Now Be Submitted Online Using FamilyID!

Attention all students in grades 7-11 who are interested in playing a Fall 2019 Sport.

Sport Physical registration will now be an online process using FamilyID. Please read the attached memo for further information on how to register and and how to submit the signed section 6 form from the Doctor.
